Who knows... When you are not able to reproduce the issue it might be very hard to root cause it. As a first step I recommend to eliminate known bugs: T_ERROR. I filed this: Bug #3910 t_look(3NSL) should never return T_ERROR https://www.illumos.org/issues/3706 Currently I do not plan to fix it myself soon, so feel free to take it and work on it. I'd be happy to review the fix (the fix shouldn't be hard).
