In my experience the best approach is to default to labels but provide users with the option to display as icons or text (see most web browser preferences for instance).
With that said, context is everything and depending on the application any number of icons can be considered intuitive (save, open, favorite, reload, delete, forward/back, search, etc.) . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .
