Websense may be a viable option. We only use the policy portion, which does auth with an LDAP backend and i know they can display usage, and after talking with our websense guy hey says there is some ability to do quotas. He also says with their network agent, or their appliance you can do some bandwidth throttling. I'm not saying Websense is the greatest, but it might fit for you.
