Bathtub curve.  Defects emerge either very early on in the lifecycle
of a system (or a batch of systems), or all at once toward the end of
the systems's planned life.

I see more "system integrator oops'ed" errors than outright hardware
failures at the beginning of a machine's life, to be honest... but you
DO see the hardware failures.
