Sign up for a free/basic Dynamic DNS account at no-ip.com
You install a small service on your machine which can watch your router
for changes to the I.P. address assigned to it by your broadband
provider. It will then update a DNS entry for you so that you can
contact the machine by a hostname.

Then you need to open some ports (3483 & 9000) on your router pointing
them to the Slimserver box. The exact mechanism for doing this varies
by router.

If you just want to stream audio to Winamp, you just need port 9000. If
want to run Softsqueeze, then you'll need 3483 also.
