Is it accompanied by heavy disc access? Then it's rescanning. (Although rescanning shouldn't use 100%, CPU, in my case it's 25-50%).
If the processor is pegged but there's no disc access, either the scan has failed or it's another process. Do you have any playlists that reference files that have been moved? I believe if you start Slim manually with the d_scan debug flag on, it might indicate where the scan is tripping up.
