Hi Atom,

We use Nagios, plus PNP (http://www.pnp4nagios.org/pnp/start) for  
converting performance data into RRDs for metrics. Most Nagios plugins  
provide performance data, and if not it's fairly trivial to add them.

I think Groundwork also does something similar (http://groundworkopensource.com 
)

There's also a Nagios plugin for Cacti that I've been considering 
(http://trac2.assembla.com/npc 
)
