Isn't Voiceover a major screen reader?
I haven't experienced this with youtube, but have noticed that voiceover occasionally doesn't find links on sites. I'm not sure if it's because the links desplay as images, so the screen reader can't read them, or what exactly the problem is. Don't have a windows machine around anymore, so haven't tested these sites with a windows screen reader.
