Thanks for pointing it out, Bjorn. The article has a fairly
comprehensive review of literature from the perspective of James
Reason's book on human error (J. Reason, Human Error, Cambridge
University Press, Cambridge, UK, 1990). Hopefully I've contributed a
few insights of my own here and there as well :)
