That's because currently the process for filling one out is a PITA - paper
form, print, sign, scan, email. We'll start pushing 'em harder once
they're more accessible (e-sign, probably).
I'm going to close this as "invalid" (even though it's clearly correct):
this is a DSF action item, and not something the core team can (or should)
be responsible for. It's on our (read: my) radar though, and will get done
shortly. Thanks for the prod!
