-14 is the first post, 2.6 months after rechartering.

* In the introduction (last paragraph) mention that the purpose is twofold, 
debug and anti-abuse.
* In Section 2 (2nd paragraph) clarify that failure reports allow better 
determining the failure reason.

Appendix B.14 (the above two lines) is on Github but is missing from the -14 
draft.
