its really not that complicated: When programming a break, not all the drum hits need be the same volume.  A real drummer sometimes hits the drum hard, sometimes soft.  A "ghost" hit is just an especially low hit- one thats so low you barely hear it, but you still kind of "feel" it (when i say barely hear it i mean the average listener- producers hear them quite clearly casue we know they are there). To learn more, take a famous break and slow it down in a wav editor to find all the ghosts. 

And DO NOT listen to all these people giving you formulas on where ghosts "should" go- they will just confuse you.  Ghosts can go ANYWHERE, depending on the "flow" you are trying to create.
