Its OK now! This was caused by a double import of "fo/docbook.xsl". It was included in two of my stylesheets (one was imported by the other). This error only showed up with XSLTPROC and not SAXON 6.5.5. One question that this rises is - Shouldn't the stock Docbook stylesheets give a warning if you include it twice?
