This looks like it would be a good fit for the Formatting Objects (FO)
stylesheet, which allows publishing to PDF. The default FO/PDF would need
adjustment, but you can certainly adjust to a very precise style
<http://www.sagehill.net/docbookxsl/PrintOutput.html>.

Did you try FO/PDF and find that it was too far from your required output?
