Hi Gerard,
It looks like all your refentry elements are contained in second-level
section elements. As such, they are treated as third-level sections,
and since the stylesheet param toc.section.depth is set to 2, they will
not be added to the TOC. Setting that param to 3 should fix it.

There are about 40 man pages embedded in the document. None are being added
to the TOC.
In most cases the refentries are a running part of book->part->chapter. They
are all also included with xinclude (not sure if that is part of the
problem).

Normally refentry titles automatically appear in the table of contents.
What form of output are you producing, and what version of the
stylesheets?
Also, how are the refentry elements contained in the document?
