#1399: generate-all-reload should realize if you have migration classes and
create
a migration table with latest value

when you run generate-all-reload that it should know if you have migration
classes and if so create the migration table with the value set to the
last migration number.
I've ran across instances multiple times where I had to go an manually and
add the migration table along with the correct value.
