#1445: Doctrine_Migration::addColumn failing quietly

I couldn't get anyone else to check if the problem affects only, but that
should be trivial to do.
When trying to add a column to a table, the operation fails without any
kind of error. If it's not a bug but a usage problem I would recategorize
this as an enhancement and request adding of error messages to the
migration functions.
