For domain transfers, I save outstanding order id numbers and I poll results from opensrs using CHECK_TRANSFER. You can wait until either a "cancelled" or "completed" status appears and take action accordingly.
As to "why" an transfer order fails, you will have to consult the notes of the order using GET_ORDER_INFO. As of right now tho, you only get the first note when using this API call.
