+ BUG: id 679963: "Class Index" appeared twice in the PDF TOC, Index at the

+ BUG: id 681281: Make default for TAB_SIZE 4 and added remark in Markdown
section of the manual about the effect of TAB_SIZE on code block
processing when using tabs in the comment block.
+ BUG: id 681023: Project logo was not included in the Qt help output.
+ BUG: id 680992: Fixed a couple of typos in the comments.
+ BUG: id 681350: Fixed a problem with Markdown processing of a @code block
inside an indented /// style comment.
+ BUG: id 679928: Disabled section level correction for Markdown pages as
is was confusing.
