The patch in DS-118 had three problems:
1. did not include Bitstream sequenceID in the URL so it would not distinguish
between two bitstreams with the same name
2. did not apply URL-escaping to bitstream name
3. another case where the bitstream URL was generated in the addReviewSection
method, wasn't fixed.
The patch attached here fixes these.
