New INSTALL event when an Item is approved

There appears to be no way to discover that a new Item has been accepted. We
can, for example, do backup more intelligently if we know that there is
something new to back up.
This change creates a new Event type INSTALL, which is fired when an Item is
installed into the repository proper. That is: when it moves from a Workspace
or Workflow to a Collection.
