I tested our 1.6.0 system with Mark Diggory's patch the trunk incorporating
fixes from https://jira.duraspace.org/browse/DS-253 and under stress, we
stopped seeing the white-screen state. We deployed the fix to production and
have been running free of the white-screen state since. The issue from the
above mentioned problem was occurring but the exception was being swallowed in
the DSpaceCocoonServletFilter, hence the white-screen and no output.
The patch to the Cocoon dependency is not maintained in DSpace SVN, and is
described in the following #duraspace log by Mark:

So my understanding is that this has been fixed on the trunk.
I also patched DSpaceCocoonServletFilter to log stack-traces of any exceptions
occurring in there, so there will at least be evidence if another issue crops
up in the same area of request processing.
