Just a correction to my previous posting on this for the list:

Actually, I've just taken a look at the version of the add-on that I
amended and it is actually for v1.4 not v1.3 - it was v1.1 of the
Request Copy add-on. However, it didn't work out of the box for v1.4.1
(due to differences in the files Satya mentioned), however, as I said in
my previous message, it was fairly easy to identify the changes made to
the files in question and apply them to the v1.4.1 versions of the
files.

Apologies for any confusion.
