I had this happening, and it was driving me crazy.  After much 
messing around, I did a clean install of OS 9 and it went away.  I 
then started adding in an extension or CP every couple of days and 
eventually found that it was the "PN60" chooser extension that was 
causing it, and I haven't had a problem since.

So it might be a piece of old/bad software.  Your mileage may vary.
