why do you use -pad? are you trying to burn cdda to a DVD?

Sounds like a media problem. have you tried different media? Have you tried a 
newer firmware? maybe the media type is newer than the firmware table?
also try specifying -force.

other than that and really making sure that hdparm -r0 -d0 is set, I don't 
have much advice for you.
