It counts only as Italy. There's a quirk in the CQWW rules. They use both the DXCC list *and* the WAEDC countries list. The WAEDC list includes Turkey (the part that's in Europe), GM/Shetlands, IT9/Sicily, and 4U1VIC. Since only the European part of Italy could be on the WAEDC list, that leaves IG9 and IH9 kind of in limbo, so CQWW added these as a separate multiplier (as well as the Asian part of Turkey).
You could check the official ARRL DXCC list at http://www.arrl.org/awards/dxcc and see for yourself that only Italy and Sardinia (IS0) are separate entities.
