Are you sure the *headset* volume is maximum? The speaker volume and headset volume are independent settings, so setting one to max won't alter the other.

But, if you set the volume to max without the headset & plug it in afterward there is no warning. I would have expected just a brief display of the warning reflecting the current volume setting.
