Yes happens every time on Ubuntu 15.04 with all updates applied. How I open the application doesn't seem to matter, as long as the application is not locked to launcher. For example, I can open the trash with "Super+T" or I can open the calculator by tapping super key and typing the name. Neither of them get a shortcut assigned.
Applications which are locked to launcher get and retain the correct shortcut always. -- You received this bug notification because you are a member of DX Packages, which is subscribed to unity in Ubuntu. However, when I open another application, the previous one gets a number assigned, but the new one does not. Moving the application's icon around in the launcher also assigns it a shortcut. Note: I have less than 10 application icons in my launcher.
