This is because you are not using the Human icon theme, and the icon
theme you are using doesn't supply any notification icons. I'm marking
this as a duplicate of #344385. The title doesn't really match but the
discussion has moved on to this issue.

Bug description:
I don't know whether it's fault of nm-applet or notify-osd, but everytime I'm
connecting, it shows notification without icon. Every time I disconnects, there
is notification with icon. I don't understand why is this. Shouldn't there also
be icon while connecting?
