David I use all 3 items that you mentioned; to be honest I never thought about it at all !!! I don't tend to worry about it. I am not sure if LOTW confirmed is an ADIF defined field ???
In DX4WIN, you will see that in File Preferences and the QSO tab, you can select "Card in Hand" to "card or uploaded" in the country window you will see the status change depending on how you have set up your preferences. Try making a spoof log with 1 QSO, setting LOTW confirmation; and check the ADIF expert when you change preferences >?
