Regarding #1, if you try to create/change the manager on the same date date as is already in the database, then the one which is alphabetically earlier in the callbook will still show. For example, if FH5TT is shown as via F5SCI, but the real manager is F5CSI, then F5CSI will still show in DX4WIN because it's earlier in the callbook.
Regarding #2, it sounds like you may not be in Expert Mode, first tab of your preferences. If you run across errors in the QSL manager database, please send me the correction(s) so I can fix them in the database.
