I have exactly the problem you describe.
I found that my Computer Registry didn't contain the
correct setup as it should. I have a note from Jack that
describes (for Windows 98) what the Registry should contain:

The registry entries for Win98 are different than the one you
quoted.  XP uses different registry structure.

For Win98, it is as listed below.  If you don't have them in your registry,
you could try manually adding the structure, or you could reinstall DXbase.
If you choose to reinstall, you should first copy your .mdb(s) to a safe
place, then install new using same callsign as your .mdb callsign.
Afterwards, copy your .mdb(s) back in and overwrite the empty one that we
installed.

The registry entries for Win95/98/ME are located as follows:
HKEY_LOCAL_MACHINE
  System
   CurrentControlSet
    Services
     VxD
      DriverX
       DXB98 COM1
       DXB98 COM2
       DXB98 COM3
