I have not tried different cables, and the issue is not caused by cables.
Moving a machine with 82599EB to the 3064, or one with X540-AT2 to the 4900,
using the same cables, is enough to trigger the issue. There are no other
other messages in system logs or on the switches that indicate other errors.
I have not tried other ports as we have a sufficiently large sample size
(12x 82599EB, 9x X540).

I assume the settings are autonegotiated, and we did not change them.
