(I've also seen this in old versions, e.g. v2.0.3)

My understanding is that Idea always saves when another app is brought
to the foreground.  Once in a while it doesn't seem to save changed
files (they still show an "*" in the tab).  The result is a command-line
make ends up running with the old copy of the file.

Maybe IDEA is occasionally missing an event to notice the context
change?  Or maybe it's busy garbage collecting, or other periodic
CPU-eating activities IDEA seems to do.  It doesn't happen often enough
for me to know for sure that this is the problem.
