It's really dependent on the company.

We have all the different formats defined in our order-to-cash system so
that all we need to do is ask if the customer wants an SOPI or SOTI
structure (Shipment-Order-Pack-Item or Shipment-Order-Tare-Item.)

A classic case in point:  I am managing an ASN change for a very large
customer right now. They currently get an SOPI ASN - a Shipment and Order
loop for each order, and a Pack and Item level loop for each item on the
truck.  They have asked us to implement an SOTI structure because they
anticipate ordering more than 500 lines per order and their system has
difficulty handling that.   

So I am implementing the SOTI.   This will be a Shipment and Order level as
before, but then a Tare loop which will have a UCC128 pallet label for each
pallet.  The pack loop is eliminated and just the items on that pallet are
sent.  I seriously wish I could see how this will help them, but that's not
my concern.  :)

When they scan the pallets, the UCC128 bar code is resolved to the data on
the tare loop of the ASN and all the items are received, rather than the
receiver having to spend time receiving each item on the pallet.   We'll be
heroes at their distribution center when this happens, as checking in one of
our trucks to their DC takes them on the average about 6-8 hours.  LOL

We are in the beginning stages of making some value added changes to our
shipping process, including IT changes of our barcode solution integrated
with our SAP system.

Question in regards to 856 requirements:

If you ship a pallet or multiple pallets containing multiple boxes, what do
most companies expect to receive on the ASN?  Do they wish to receive the
number of pallets, number of boxes per pallet, box id including # (box 1 of
20), qty in each box?  Should each pallet have it's own id # such as pallet
# 1, # 2, etc?

We are in the electronics manufacturing industry so the same business
process may or may not apply to multiple industries.  Though, I appreciate
anyone's input.
