How many concurrent users do you have behind EFW? Do you use
NTLM-based authentication? How many authentication children processes
is Squid starting?

I had "zero sized reply" problem, but don't know if my case has
anything related. We use NTLM auth with a local DC (backup DC is a
remote one) and approx. 100 concurrent users. Thanks to a
configuration mistake, Squid was configured to use remote DC as
primary and 50 authentication processes -- that way, users were given
either a "zero sized reply" or an authentication box. Changing
authentication settings to local DC and raising authentication
processes to a higher number (around 120, I think) solved the problem.
