In general, you select a gateway service to proxy your requests to a
back-end processor which transfers the funds from a consumer to a
merchant.  Merchants pay monthly and or per-transaction fees to the
gateway service and the back-end processor, in addition to card-issuer
discount rates.  It's a fairly deep subject with lots of smoke and
mirrors.
