Jerry,

I had exactly the same problem. The culprit is probably the small choke
RFC15 under U4. When you bend the leads to take up that odd form between the
pins, it's very easy to break the internal connection, and the effect is
just as you describe. If you haven't got the correct value handy, try any
small fixed choke just for testing.
