If this would be unacceptable, then a less radical departure from the current Emacs Info situation would be to recognize titles by the top of the node pattern:
How about recognizing them based on the number of characters in each line? For a title or heading, you have a certain number of characters of text, and the following line has the same number of characters, all the same, and they are one of the fixed set used for "underlining". If filling leaves an ellipsis on a line by itself, the previous line will be a lot more than 3 characters long. So this criterion will not match.
