add "finally", and other exception improvements
closes #364, closes #988
rename jl_raise to jl_throw to match throw() in julia
throw() initiates a new exception, add rethrow() to continue propagating
now we record backtrace data on every throw(), but only do the much more
expensive part when backtrace() is called later
