- Empty table of contents

Export works fine with a full-buffer export. Exporting a subtree to ASCII (C-@ C-ce A) seems be fine too.

The pdflatex output has some other issues I haven't
fully identified by visual inspection alone.

I did revert to
25226d7d65e4620cbe1b814b85226547cd2642eb, and the output is
fine with that tree.
