I think you can differ between the data entry platform and the platform where you analyze the data. You do have to train people using Emacs/Org-mode as well. When I was a researcher, I used SurveyMonkey for my surveys. It's open source and web-based. You get the data in CSV files AFAIR. I also tested Google Drive (spreadsheet) forms which were very easy to set up. However, the person filling out the survey needs a Google Drive account. The results were exported in various formats AFAIR.
