Follow-up Comment #1:
I think I have fixed this. Previously I had rewritten the appointment
schedule code to simplify it, but the old method turns out to be more
reliable, so I have reverted it.
While looking at this bug, I also found a small bug in the handling of
forthcoming appointments, which I have fixed as well.
