AOL uses a proprietary email system which pre-dates the ISP boom. When AOL users started demanding the ability to send messages outside the AOL network to their friends at other ISP's, AOL was forced to patch their email system to include an Internet email gateway. Apparently, the gateway isn't perfect.

That's difficult to say. One possibility is that it would be too difficult to implement Internet email standards in the current AOL email environment, and this is the best that could be done -- short of rewriting the entire AOL email system architecture. Another possibility is that this is an intentional effort to discourage people from leaving AOL, since it gives the false impression that only AOL email is 100% reliable and that going anywhere else would jeopardize your ability to email your friends and family. I suppose only AOL management knows the real answer.
