I did a very unfinished port of emc to -rt a few years ago.

as I see it, this needs an interested programmer to spend time on it;
then we can know the answer with certainty.
One roadblock to doing a userspace system based on rtapi_app is that we
use linux kernel APIs to do stuff like enumerate hardware (hm2). To use
an -rt userspace system, we would have to add a much larger "device" API
to rtapi for these purposes. (Of course, a module-based system would
also be possibe under -rt, or at least I don't know why it wouldn't be.)
Another problem is simply that the developers simply don't have the time
to make more emc packages. This can only happen with new committments
from members of the community.
Finally, unless -rt is absolutely as good as RTAI, we will create
confusion among the vast majority of our users (parport software step
configurations) who will end up with a 50/50 chance of downloading the
wrong thing altogether.
