Where in the grand scheme of things are tool offsets applied?

There have been two enquiries on the forum recently about adapting for
tool wear in real-time. One, I think, was using a die-grinder in a
milling style of machining, so needed adaptive tool-radius
compensation (Or, at least, he thought he did). The second has not
been too clear about his requirements yet.
Are tool-offsets applied in the TP, and "baked in" to the lookahead
(such that real-time changes are not possible) or is it part of the
kinematics area, such that it might be plausible to change them in
real time?
