So far, this is only about offsets (what we used to call "tool
length offset" back when it was only in Z), not diameter.

G42.2 D100

I don't really like how clumsy that is.

Sometimes I wish we could specify multiple words with the same
letter, like G42 D1 D100 D101.  Some controls let you do that (my
BOSS8 used it a lot with canned cycles), but not ours.

G43 H1 H100 H101 feels a lot better, too.
