Hi; Misconnection is one of the standard "abnormals" tests performed for safety agency compliance. Check your own applicable standards. I believe CSA specifically states that connections must be attempted whenever a plug could be fitted to a receptacle. At least around here, they are absolutely certain (e.g. during "witness" testing) to ask us to plug "telephone" type of plugs into telephone jacks. But I am fairly sure there is no standard that will provide a hard-copy list of specific misconnections. No matter how carefully you draw up such a list, there would always be loopholes.
On the other hand, it shouldn't be too hard to come up with a scheme that would open a fuse or something if somebody made such a blunder.
