Alan - I'm not certain about the origin of the application of Tables 3, 4 and 5 in your example, but since Tables 3, 4 and 5 generally have lesser distances for the same voltages than are found in Table 6, this seems primarily an academic exercise for which I'd need a specific safety concern to pursue.
