As far as I know, the only certification house that
requires a production-line hi-pot test of TNV circuits
is UL. The test is specified in the gray pages as a
separate test from that of the mains hi-pot test.
Since you mentioned "primary wiring" in your quote
of the UL requirements, I wonder if you are reading
the correct gray page.
The UL test is from tip and ring, connected together,
to ground.
In the TNV circuits I am familiar with, there are no
overvoltage components, e.g., sidactor, from tip or
ring to ground. Instead, the sidactor is connected
between tip and ring. I don't know of any hi-pot
test requirement for the insulation between tip and
ring.
